A few days after an operation, many patients ask themselves the same questions: why have I swollen so much? Why does the pain come back as soon as I move? How do I sleep comfortably with a shoulder, knee or hip that pulls? And above all: how can I get through this phase faster, and more calmly?

This article sheds light on what really happens in your body after surgery, and on how cryocompression, controlled cold combined with dynamic intermittent compression, can support your recovery. What happens in your body after surgery?

An operation, even well performed and minimally invasive, remains a traumatic event for the local tissues. To understand how cryocompression works, we first need to look at what the body triggers naturally after a surgical procedure.

Inflammation: a normal but uncomfortable reaction

Inflammation is the body's first response. It is useful and necessary: it brings the cells that will clean and repair the operated area. But it also comes with unpleasant effects:

  • Swelling (oedema): fluid accumulates around the operated area
  • Local warmth: blood flow increases at the site
  • Redness: a visible sign of this increased circulation
  • Pain: the nerve endings are stimulated

This inflammation usually peaks between the 3rd and 5th day after the operation, then gradually decreases. It is often at this point that patients find the period most difficult.

Swelling and stiffness: a vicious circle

Swelling is not just a cosmetic detail. When a joint swells, several things happen:

  • Range of motion is mechanically limited
  • The muscles around the joint go dormant (sometimes called muscle inhibition)
  • Pain increases at the slightest movement
  • Sleep becomes more difficult, which slows recovery

This vicious circle is one of the reasons your surgeon and physiotherapist insist so much on managing swelling early.

Pain: a signal to respect

After surgery, pain has two components: so-called nociceptive pain (signals sent by damaged tissues) and inflammatory pain (linked to swelling and pressure on neighbouring structures). Reducing inflammation and swelling also acts on a large part of the pain.

Cold: why it helps

The idea of applying cold to a painful area is not new. What has evolved is our precise understanding of what cold does to the tissues, and how to apply it in a controlled, sustained way.

Vasoconstriction: less swelling, less inflammation

When you cool an area of the body, the blood vessels narrow (vasoconstriction). In practice:

  • Less blood reaches the operated area
  • Less fluid leaks into the tissues
  • Swelling builds up more slowly

This is one of the reasons cold is used straight out of the operating theatre in most modern surgical protocols.

Slowing of pain signals

Cold also acts directly on the peripheral nerves. It slows the speed at which they transmit pain signals to the brain. It is a temporary effect, but one that can offer real relief, particularly at bedtime or before a mobilisation session.

Reduced local metabolism

Cold reduces the metabolism of the cells in the cooled area. In practice, this limits the inflammatory cascade and the production of certain substances that amplify pain. It is a more discreet but well-documented effect.

The trap of the ice pack

An ice pack placed on a joint has several limitations:

  • The cold is too intense at first (risk of a cold burn)
  • It warms up quickly and becomes ineffective after a few minutes
  • It does not adapt to the shape of the joint
  • It only provides cold, without acting on drainage

A cryocompression device maintains a stable, controlled cold, at 4°C, which corresponds to the range where the benefit is maximal without risk of skin damage. The difference in comfort and effectiveness is significant, especially over time.

Compression: why it helps

The second pillar of cryocompression is dynamic intermittent compression. To understand its value, we need to look at how fluids circulate in your body.

Venous return: bringing the blood back up

Your veins bring blood back towards the heart. In the limbs, this upward flow happens in particular thanks to muscle contraction, which acts like a pump. After an operation, you move less: the muscle pump works in slow motion, and blood stagnates more easily.

Rhythmic compression simulates this muscle pump. With each cycle, it gently pushes blood and fluids towards the trunk, which reduces stagnation, helps decrease swelling and improves tissue oxygenation.

Lymphatic drainage: clearing the swelling

Alongside the venous system, the lymphatic system collects the excess fluid that builds up in the tissues. When an area is inflamed, this system can be overwhelmed: that is the visible swelling. Intermittent compression supports lymphatic drainage, which helps clear the excess fluid, particularly useful in the first week after the operation.

Why "intermittent" rather than continuous

Continuous compression can be counterproductive: it can hinder circulation instead of helping it. Intermittent compression alternates phases of pressure and release, reproducing the natural rhythm of venous return. This is what makes the effect more physiological and better tolerated.

Why cold + compression together?

Taken separately, cold and compression each have their effects. Combined, they work in synergy: cold limits inflammation and pain, compression clears the swelling already present. It is this combination that sets a cryocompression device apart from a simple ice pack.

What the studies show

Cryocompression has been the subject of several clinical studies in recent years. Here is an overview of the benefits observed. These figures relate to specific surgical contexts and do not apply mechanically to every situation, but they give an idea of the orders of magnitude.

In detail:

  • Shoulder surgery: across 200 patients in 5 centres, cryocompression reduced the need for opioids by around 50% in the first days.1
  • Knee replacement: a randomised trial showed a reduction in knee circumference (about 2.7 cm less) and faster improvement in pain on activity and function compared with standard cryotherapy.2
  • Back surgery (scoliosis): after scoliosis surgery in children and adolescents, morphine consumption was reduced by 29 to 52% depending on the day.3

These studies do not say that cryocompression replaces medical treatment. They suggest that it can be a useful complement to the overall protocol set by your medical team.

  • It is not a substitute for physiotherapy. Rehabilitation remains central to recovery.
  • It is not a replacement for the medication prescribed by your doctor. Follow the protocol you have been given.
  • It is not suitable for every situation. Some contraindications exist (severe circulatory disorders, problems with skin sensation, etc.). Your surgeon will confirm whether it is appropriate in your case.
